WELCOME CLUB 2008-2009 VOLUNTEER SIGN-UP FORM

Community service is the foundation of our club, therefore, we would like to invite you to sign up for our yearly activities. Please remember that any amount of time that you are able to give is always appreciated! Before each event, you will get a reminder phone call or email from the person in charge of that event. Please fill this out and let us support our community together!

1. SERVICES ­ We collect and distribute food, clothing, and other items to the needy within our community. Please check the ones that you will support with donations. If one that you are interested in is not listed, please use "other".

2. Adopt-A-Highway ­ we clean a two mile section of Route 309, just past St. Jude's Church/School twice a year. The more helping hands, the faster the job gets done. Babysitting will be provided.

3. General Meeting ­ Volunteers are needed to prepare desserts, and to make/buy centerpieces for meetings in September, October, November, January, February, March & April. If there is no centerpiece, we will do a 50/50 drawing.

4. Meals that Heal ­ We prepare and deliver meals to families that have had a baby, an illness, an accident, a death, or another lifestyle event.

5. Relay For Life - Relay For Life is the American Cancer Society's signature activity. In Mountain Top, teams of people camp out at Crestwood High School and take turns walking or running around the track. Each team is asked to have a representative on the track at all times during the event. Relay is an overnight event, up to 24 hours in length. All proceeds go to the American Cancer Society. The Relay will be held in June 2009.
